At Hazelcare Limited, we provide a unique supported living service, Our supported living model requires a person-centered approach to how support is delivered to individuals (REACH Standards).

Each person will have an assessment, which will identify the level of support that they require. Hazelcare Limited will work with the person they will support to ensure that they have a structured day and support them to develop the skills to increase their independence, this may be very simple steps at first.

Hazelcare Limited’s supported living services are regulated by the Care Quality Commission (CQC),. They are also overseen by our own internal quality teams, ensuring a robust and responsive approach that means that we support people in the best possible way.

Our care team specialise in working with individuals with learning disabilities. Including individuals with autism spectrum disorder (ASD), Down Syndrome and Prader-Willi syndrome (PWS). We also offer services to those with any mental health needs, those with brain injuries and those who present behaviours considered challenging. We work in a person-centred way, which means that our support and care is completely bespoke, and perfectly matched to the unique needs, preferences and aspirations of each individual.

We work closely with landlords to ensure that our residents receive the highest standards of care and support within housing accommodation. Our staff teams have significant expertise in supporting people with complex needs and include dedicated positive behaviour support (PBS) specialists .Our staff have experience in CBT , DBT , Structured Clinical Framework .

The Reach Standards are also embedded across our supported living services. Our aim is to empower individuals to make their own choices about how they live their lives, but to be supported to do that when that may become difficult. Our staff take the time to really get to know each person and to understand their needs, interests and goals. They provide the care and support which sees residents improve their wellbeing and independence and achieve their potential.

Supported Livng

Hazelcare Limited has established partnerships with housing providers and we are proud of the quality and range of our accommodation schemes. Our partners offer self-ontained apartments, bungalows, shared living and completely bespoke solutions, with accommodation adapted to meet individual needs.

All of our tenants’ homes are comfortable and modern, with assistive technology services incorporated into the buildings if required to maximise independence for residents. Our supported living services are located within communities, so they have easy access to amenities and transport links.

By understanding each person’s needs and aspirations, we can agree together the type of accommodation and locations that might work best for them. We can provide help with tenancy agreements where needed, giving people the security of their own homes. If an individual wishes to live with others, we match them with people who have shared interests and lifestyles.
