We provide supported living offering a full range of high-quality supported living services.
This service supports residents with mental illnesses, complex trauma and learning difficulties. With support from a team we will offer individualised support through a personalised support plan reflective of our resident’s needs incorporating the views of carers and relatives.
The hours of support vary depending on individualised plans, the service is 24hr support service with a variation of staff and end of the phone and touch system that will ensure easy access for response or support.
We use innovative technology for outcome measures and it support service users, stakeholders and the local authority to deliver a powerful combination of time / attendance data, progress towards outcomes and lone working and support. This ensures the right care is delivered at the right time and ensuring we can account for the care and provision of quality care. Our service ensures that we safely manage risk of residents, and there will be a 24hr response service with an on-call system to ensure a safe service that provides support.
Model of Care

This ensures that people who use the service can live as full a life as possible and achieve the best possible outcomes. The principles reflect the need for people with learning disabilities and/or autism to live meaningful lives that include control, choice, and independence. People using the service receive planned and co-ordinated person-centred support that is appropriate and inclusive for them.
